St. Luke's Medical Center, Makati Medical Center, and The Medical City are top private emergency facilities in Metro Manila. For island emergencies, Philippine Coast Guard (hotline: 1070) coordinates maritime medical evacuations.
Response time: 10-25 minutes in Metro Manila; 20-60+ minutes in provincial cities; remote islands may require boat/helicopter evacuation (hours). Traffic in Metro Manila (especially EDSA corridor) can significantly delay ambulance response.

Healthcare System
The Philippines has a dual-track system: private hospitals in Metro Manila and Cebu offer excellent, internationally-trained care (many Filipino doctors trained in the US), while provincial and public facilities are under-resourced. English is widely spoken — a MAJOR advantage for foreign patients, making the Philippines one of the easiest Southeast Asian countries for healthcare communication. The country is a growing medical tourism destination, especially for dental, cosmetic surgery, and cardiac care. PhilHealth provides basic coverage for citizens but co-payments are significant. The 7,641-island geography creates enormous access disparities — Manila has world-class hospitals, while remote islands may have only a rural health unit (RHU) with a nurse or midwife.

Medication
Medications are very affordable at Philippine pharmacies. Mercury Drug (largest chain — 1,000+ branches nationwide) is reliable, well-stocked, and open late. Generic medications (Republic Act 9502 — Cheaper Medicines Act) available at 50-80% less than branded versions. Antibiotics: PHP 50-300 ($0.90-5). Common medications: PHP 20-200 ($0.35-3.50). Pharmacists can recommend treatments and supply many medications without prescription. Branded US/European medications also available but at higher prices. TGP (The Generics Pharmacy) chain specialises in affordable generics.
Pharmacy & Medication
Hours: Mercury Drug: generally 7am-10pm daily, many locations 24/7. Watsons: 10am-9pm (mall hours). TGP (The Generics Pharmacy): 8am-9pm. Rose Pharmacy (Visayas/Mindanao): 8am-9pm. Hospital pharmacies: 24/7.
OTC: Very wide range — antibiotics, anti-inflammatories, and many medications available without prescription at pharmacist discretion. Pharmacists are knowledgeable and will consult on minor conditions. Prescription technically required for some drugs but loosely enforced.
Controlled substances: The Philippines has EXTREMELY strict drug laws — the Duterte-era anti-drug campaign resulted in severe enforcement. ANY amount of illegal drugs carries heavy penalties. Cannabis is illegal — zero tolerance. Carry doctor's letter for ALL controlled medications including opioid painkillers, ADHD medications, and benzodiazepines. Declare at customs if in doubt.
