Call 112 for all emergencies. INEM (Instituto Nacional de Emergência Médica) provides ambulance and helicopter MEDEVAC service. Before going to the ER, call SNS 24 (808 24 24 24) — they can triage, advise, and provide a referral code that waives the €20 ER fee and fast-tracks you. ER wait times at public hospitals (especially in Lisbon) can be 3-8+ hours for non-urgent cases.
Response time: 10-20 minutes urban (Lisbon, Porto). 20-40+ minutes in rural Alentejo, interior, and Azores/Madeira islands.
Cost: Public ER (urgência): €20 taxa moderadora (moderating fee) — waived with SNS 24 referral, EHIC holders, pregnant women, children under 18, and low-income users. Private ER (CUF, Lusíadas): €100-300. Ambulance (INEM): free.
Healthcare System
Portugal's Serviço Nacional de Saúde (SNS) provides universal healthcare but faces a severe GP crisis — over 1.6 million residents lack a family doctor (médico de família). The system is strained: public GP waits can be weeks to months, and ER departments are frequently overwhelmed. However, private healthcare is affordable, high-quality, and widely available — private GP consultations cost just €40-80 and are available same-day. Portugal has become a major expat destination (D7, Digital Nomad, Golden Visa) and the healthcare infrastructure in Lisbon, Porto, and Algarve is modern. SNS 24 (808 24 24 24) is a 24/7 triage helpline that can direct you to the right level of care and even issue "green receipts" (vale de consulta) for priority SNS appointments.

GP Access & Costs
Public: Centro de saúde (health centre) with USF (Unidade de Saúde Familiar) for registered patients. Over 1.6 million residents lack a médico de família — visitors have even less access to public GPs. Private clinics widely available and affordable: CUF, Lusíadas, Hospital da Luz, Joaquim Chaves. Walk-in private clinics in Lisbon, Porto, and Algarve.
Typical wait: Public GP: weeks to months (severe shortage, varies by region). Private GP: same-day to 2 days. SNS 24 triage: immediate phone consultation. Specialist (public): 2-12+ months. Specialist (private): 1-2 weeks.
Availability: Severe GP shortage nationwide — 1.6+ million residents without a family doctor (2024 data). Lisbon, Porto, and Algarve have the best private options. Rural interior (Alentejo, Trás-os-Montes) and islands (Azores, Madeira) have significant shortages. Private chains CUF, Lusíadas, and Hospital da Luz offer reliable access in major cities.

Public (with registration): €5 taxa moderadora
Private GP
€40-80 per private GP consultation. Private specialist: €60-150. CUF Descobertas (Lisbon flagship): €60-100 GP. Private insurance (Médis, Multicare, AdvanceCare): €40-120/month.
ER Without Insurance
Public ER: €20 taxa moderadora (often waived). Private ER: €100-300. Hospital stay (public): free for registered users. Hospital stay (private): €200-500+/day.
Medication
Medications are affordable — generics widely available and encouraged (pharmacists must offer generic alternatives). Many medications cheaper than Northern/Western Europe. SNS subsidises medications for registered users (up to 90% for essential drugs). Visitors pay full pharmacy price. Prescription medications require a receita médica.

Pharmacy & Medication
Hours: Mon-Fri 9am-7pm, Sat 9am-1pm. Farmácias de serviço (duty pharmacies) operate 24/7 on rotation — check the green cross sign or farmaciasdeservico.net. Every pharmacy displays the nearest open duty pharmacy in its window.
OTC: Portuguese pharmacists are highly trained and approachable — excellent first stops for minor ailments. Can advise on medications, measure blood pressure, and provide basic health checks. Parafarmácias (in shopping centres) sell non-prescription products at lower prices. Some medications available OTC that would require prescriptions elsewhere.
Controlled substances: Portugal famously decriminalised personal drug possession in 2001, but this applies ONLY to personal use — medical regulations for controlled substances remain strict. Carry medications in original packaging with a doctor's letter. Schengen certificate needed for controlled medications. Cannabis-based medications require special authorisation (Infarmed).

- Get your NIF (tax number) immediately — apply at Finanças office or online via Portal das Finanças. Without it, you cannot register for healthcare.
- Use private clinics for faster access — CUF, Lusíadas, Hospital da Luz, and Joaquim Chaves are reliable, affordable chains.
- Call SNS 24 (808 24 24 24) before going to ER — they triage, advise, and can waive the €20 ER fee with a referral code.
- Pharmacies (green cross sign) are excellent first stops — pharmacists are knowledgeable and can treat many minor conditions.
- Check farmaciasdeservico.net for the nearest 24/7 duty pharmacy.
- Apply for SNS user number (Número de Utente) at your local centro de saúde if staying longer term — gives access to subsidised public care.
- Learn basic Portuguese medical phrases — "Preciso de um médico" (I need a doctor), "Dói-me aqui" (It hurts here), "Sou alérgico a..." (I'm allergic to...)
